Source Database Technology has developed an open source architecture Smart Form© database application for the purposes of eliminating the current paper process of records management in the hospital medical records industry.
One of the most recurring costs in record management is the process and storage of paper records. To eliminate the paper process is the main focus of the application. Paper should only be necessary as an option only in the records management process.

For example, look at the submission form for the General Consent for Medical Treatment. It contains a minimum of 5 paper pages in total. The same form is filled out by the prospective patients each and every time a patient is registered for medical treatment. This process and all of the related paperwork is repeated over and over on a daily basis. Over a period of time there are enough paper records on admittance records alone to fill a small warehouse of several thousand square feet.
Now taking a further look into the situation, it requires people to maintain these records, inventory them and locate these records and transport the records to a person or physician who may request the patients records. Maybe even copies of the originals have to be made to send out on a records request, in order to protect the originals and to keep them in original form and condition.
Using SDT's Smart Form© database system, the originals are in a digitized format held on a secure inter-relational database server. Filled in by the patient and stored in real time on the database, "No" room for errors or do overs or lost paperwork. Once the initial form is completed and submitted it's there permanently and cannot be altered or erased by anyone but the Hospital's Database Administrator.
A basic SDT application can contain over 200,000,000 records of these types and many others including PACS images as attachments and all can be queried or searched and be brought up in a matter of milliseconds. Each patient would have their own medical history from admittance to release stored in a digitized format.
It has been reviewed by Georgia State Government that the e-Solution provided by SDT can increase the efficiency of state government by 80% and save the State of Georgia $137,635,000.00 per year in the records management area alone. Not only enhanced availability of researching and recovery of records but also the security features that SDT has incorporated into it's database schemes.
Any SDT solutions user whether the front end user [patient being addmitted] or the hospitals administration benefit from the application. Physicians can login and access patient's records in real time from remote locations and consult with one another in real time. This includes, mri, cat scans and other digital information technology.
